Services de santé de Chapleau Health Services (SSCHS) is an accredited integrated healthcare provider, in the rural township of Chapleau located in Northern Ontario. The organization is comprised of a 13 bed acute care General Hospital; a Nursing Station; a variety of Community Services, including adult mental health and addictions; a 23 unit apartment complex for seniors; and a 25 bed long-term care facility with 2 respite beds, 4 chronic care beds and 19 long-term care beds. Chapleau has a population of approximately 1,964 (2016 census) and is nestled in the heart of the Canadian Shield with access to provincial parks and year-round recreational activities. About the Position:

Experience working in a primary care setting with a collaborative interdisciplinary team of allied health professionals to deliver a patient-centered model of care. The successful candidate will function in a primary care setting that will focus on health promotion, chronic disease management and continuity of care as they respond to the needs of our local community and surrounding areas.
